Biobanking Patient Consent Forms

Consent summary form

The consent summary form is a short descriptive version of the consent forms

Adult Consent Forms:

  • Allows collection of biological specimens (tissues and biological fluids) to be banked
  • The adult consent form is also available in Spanish


Parental Permission Forms:

  • Allows the collection of biological specimens from patients under 18 years of age
  • If the patient is between 7 and 11 years old, the person obtaining consent must check the box ascertaining that he/she obtained permission from the patient during the assent discussion
  • If the patient is between 12 and 17 years old, an assent form must be filled by both the patient and the person obtaining consent and sent alongside the parental permission form to the BioBank 

Assent Forms:

  • Allows patients 7 to 17 years of age to assent to the procedure
  • THIS FORM MUST BE ACCOMPANIED BY THE PARENTAL PERMISSION FORM (see above)

     

All forms above MUST be signed and dated by both the patient (or the patient's parent or legal guardian) and the person who has obtained the consent, and:
